I cant comment specifically on this. However Being a doctor in either environment will present its own challenges it will probably make more of a difference as to what field you want to go into such as emergency medicine or a specialist subject.
Army doctors do have the extra challenges of working remotely and in slightly different environments compared to there NHS counterparts. But different people will find different things more challenging than others so this is more likely to be an individual answer than a generic answer.
